So...what is the verdict for the best all-season (Midwest) tire for above model?...Nitto 420s, and if so, what size on the rears/... or the Toyo proxes?...and once again, what size on the rears (285, 295, 315)?

Stock size
Fronts 255/45/20
Rears 285/40/20
Yup Nittos in those sizes.

Just don't buy a set of OEM goodyears... no good.
x2 the goodyears suck and many have have them fall apart

Or you could try to Continental Extreme Contact DWS(Dry, Wet, Snow) in 275/40 and 315/35. According to Tirerack they kick ass in the snow.

I think that Travis and Kevin paid around $1200 with road hazzard mounted at Tire Rack. If I was keeping my truck I was gonna get them this spring.
