2004 WJ Overland Death Wobble

Hi all,
For about a year now, I have been getting the death wobble on my Jeep. It seems that the only times that it occurs is when I am braking or when I hit a bump. It doesn't happen when i am going a certain speed which I have heard that it does happen in some cases. I was just wondering if anyone had any suggestions for me to try and fix the issue. It might be the track bar but i am not completely sure. I did replace my upper and lower control arms about a year an a half ago. If anyone has anything to try and point me in the right direction I would greatly appreciate it.

Feel with your hands for play on all the steering rod ends and ball joints, while someone else will rock the steering wheel left/right. If there is play on anyone of them, you can feel it.
