I saw a really slick one that was a leather slide loader, more of a stiff pouch or wallet style with an open edge so that the phone would come out easily. It mounted to the side of the center stack by your right knee and had an opening for the charger cable. Unfortunately, I spent 15 minutes looking for it online before posting this message and could not locate it. I recall the ad for it in Macworld magazine and it was a premium product. Will keep looking.
I have found that in an Apple bumper protector, my phone stays planted on the leather dash area while driving in normal conditions, if I just lay it flat there. It also works to put it into the door handle. I usually put it in the cup holder but on long trips I'll connect it to the power port in the center stack cubby and just leave it there out of site. It's connected via BT anyway so it's not that important for me to see it or handle it on a long drive.
I'll let you know if I find that other item I was mentioning, but there are lots of similar sideloading products with flaps that could be easily modded and attached with double-sided tape or velcro.
