At the beginning, opposing traffic is failing to yield to the drivers with the left turn arrows. A driver doesn't understand how all-way stops work near the end.
We took a little tour of Christmas lights last weekend around our old houses like we do almost every year. In the span of about an hour I was nearly side-swiped by someone who didn't notice that there were two left-turn lanes at the T instersections, two drivers who entered intersections a few seconds after their light turned red, and one driver who pulled out in front of me ignoring the stop sign at the end of the mini-mall driveway while entering an arterial street with a 45 MPH speed limit.
