My key fell out of my pocket under the seat and it would not lock. Also, I have tried to start with my wife holding the fob outside the driver door and it won't start. The vehicle has to know if the key is in or out of the vehicle. Try starting it and then get out of the vehicle. The dash will indicate that the fob has left the vehicle and begin dinging.

No.
The system can detect the key in the car or outside. That is a safety feature. If the key is in the car (with you) and the doors are locked then someone else cannot come up to the vehicle and simply open the door (e.g. a thief, carjacker...). In our cases the car only detects the key fob relative to the car... not if a human is actually with it.

started car left fob in vehicle went back into work, and found out jeep is locked and running with fob IN vehicle. wth...had to call roadside assistance.

Yes the doors lock. (You always have manual control over those systems). Our Overland is set to auto lock doors when vehicle reaches about 10 mph. When you stop you can grab the inside release handle on the driver door and the driver door will auto unlock. We have it set that unlocking is a two step process. driver door is first, other doors on second click.
If you are outside the vehicle you can push a button on the handle that will lock all doors. If you grab the handle the driver door... the driver door will unlock. If you grab the front passenger door on the outside, all doors unlock. If the key fob is inside the vehicle the control dynamics change because the vehicle is set to assume you are in the car with the fob.

If you approach the key fob scenarios from a safety perspective then you'll guess which situations can cause you to get locked out. But the simple solution is to keep the fob attached to you somewhere.

Isn't there a situation where if you leave the fob in the car and then try to lock the door the horn will beep 3x and the door will unlock?

Yup.. But in my case my key was in the cup holder and it locked by itself.

Pressing the lock switch on the arm rest and closing the door with the remote in the
car is supposed to cause the door(s) to unlock along with the lights and/or horn
being activated.

If the auto start feature is attempted with the door open, it will not start the car but it will lock the doors then later if you put the keys in the car and close the door with the engine running or not, the doors will remain locked unless they are unlocked manually. I'm sure there are other situations to make this happen though. Repeated locking attempts for instance.

That's what mine does. I've done it a couple of times. I always lock it that way. Today I got out, locked it and before I closed the door I realized I forgot something. Sat back down and closed the door while I dug for what I needed and suddenly it's honking at me. Took me a second to remember why.

So my wife just locked herself out and I had to go rescue her. We were able to figure out a reproducible procedure that will lock you out.

1. Start inside the car with the key. Lock the doors using the armrest.
2. Open door, press same side armrest lock again (door unlocks when opened)
3. Close the door. No 3 beeps. No auto unlocking.

This works from either the passengers side or the drivers side as long as both presses are are on the same door armrest. Doesn't seem to work from side to side or outside.

Now, in case you are wondering my elderly mother in law was confused, hit the lock button before getting out, then while holding the armrest she hit the button accidentally a second time while getting out. Boom. Locked Jeep, key fob in the cup holder.
