Well I still haven't solved the original rattle, but at least I solved the second (and much worse) rattle.

I got lucky. I was able to remove from the bottom using only the tool you see here. Good thing, too, because it was noticeable.

I should note that I tried to recreate it this evening, and it seemed fine. My memory is that it was in park, but same question if you've experienced in neutral.

In my experience, running a car designed for 93 on 87 can impact mileage/performance as a result of the retarding of the timing (which keeps the engine from knocking and allows it to still run okay)....

As for transmission as cause - the service manager was confident that to the extent there was/is a noise, it was gear whine from the transmission.

I'm still waiting on it to do it again. My commute is only about 25 min, with 10 min or so being not on an interstate, so perhaps it needs more than that. Or maybe it's cured (a guy can dream).
